Order a MAP OTC Pack for Your Medical Mission Trip

MAP International’s signature OTC Pack is ideal for those traveling without a health care professional and contains some of the most requested nonprescription medicines and supplies, including pain and fever reducers, topical antiseptics, oral rehydration formulas, and gloves.

To apply for a MAP OTC Pack, please complete steps 1 and 2 below.

Steps to Ordering a MAP OTC Pack


1. Fill out and submit the MAP Eligibility Form.

The MAP eligibility form is a required document used to verify that your medical mission trip qualifies for receiving MAP product. This form includes questions about yourself, your organization, and your planned medical mission trip.

2. Fill out and submit your order for an OTC Pack

Once your Eligibility Form has been completed, please fill out your order for an OTC pack. To complete the OTC Pack order form, you must download or print the form below. Then you can send back by scanning or emailing to orders@map.org.

3. Wait for your Eligibility Form and OTC Pack order to be reviewed.

Once the MAP Medical Missions Team has received BOTH your Eligibility Form AND your OTC Pack order, your order will be reviewed for approval.

4. Once your application has been reviewed for approval, you will be contacted with the next steps

The MAP Medical Missions team will contact you with any questions about your application. Once approved, they will share with you the next steps in processing your order.
